Lead Family Coordinator
Salary Range: $48,000-$52,000 Yearly Lead Family Coordinator Reports to: Project Director – Systems of Care Location: 1603 Court Street, Syracuse, NY 13208 Schedule Flexible to Meet Program Needs The people we care for come from all walks of life, and so do we. Main Function: Under the direction of the Project Director and assigned to work on behalf of Onondaga County, assist with the further development of the family-driven care movement within Onondaga County and the system of care. Responsibilities include work collaboratively with Onondaga County, build peer support providers, facilitate parent and caregiver education and support groups, train partner organizations, and facilitate monthly meetings with community-based organizations providing HCBS Medicaid Waiver and NYS OMH CFTSS services, and act as spokesperson for the System of Care. Duties: 1. Assist with development and implementation of project + Build coordinated effort of peer support providers in Onondaga County + Facilitate parent and caregiver education and support groups + Prescriptively use personal experience raising a child in the mental health of developmental disability system as an engagement tool + Train partner organizations on the importance and benefit of providing services within a family-driven care framework + Facilitate monthly meetings with community-based agencies providing Home and Community Based Services Medicaid Waiver and NYS OMH Child and Family Treatment and Support Services (CFTSS) to enhance service delivery to youth with co-occurring disorders + Increase leadership skills of caregivers + Design and implement Countywide Peer Learning Collaborative + Act as a community spokesperson for the System of Care + Work in partnership with Project Director in all levels of decision making including the development, implementation and evaluation of the System of Care 2. Maintain appropriate records and prepare necessary reports + Complete required data collection and/or reports as directed by supervisor 3. Follows Agency standards and procedures + Follow agency standards and procedures + Maintains compliance with applicable rules and regulations + Provides family-centered, quality services 4. Other duties as assigned Education & Experience: High school diploma/GED required. Associate degree preferred. Lived experience navigating the system as the primary caretaker for a child with mental health needs preferred. Credentialed as a Family Peer Advocate through NYS with two years’ experience providing peer support services preferred. Familiarity with trauma-informed care, as well as cultures and languages of population preferred. Bilingual speaker preferred. Conditions of Employment: Must possess a valid NYS Driver’s license and have transportation necessary to meet departmental needs.
