Short Description

The Lead Nurse Navigator is an integral part of the Structural Heart Multidisciplinary Team.

The Lead NN supports, educates, identifies barriers to care and assists patients and families throughout their Structural Heart related diagnoses. This role helps patients navigate the complexity of the healthcare system and contributes to improved patient outcomes and patient and provider satisfaction. In addition, this position is responsible for the management and mentorship of Nurse Navigators working in the Structural Heart program.

 

Manages Nurse Navigator team in SH and is a Clinical Resource

Connects with patients of SH after initial evaluation and prior to surgical intervention.

Facilitates the Multidisciplinary Care within SH and Cooper University Health Care

Supports, educates and removes barriers to ensure timely specialized care

Assesses patients for optimization and risk prior to surgical intervention

Coordinates Disease Specific care for SH patients 

Complies with all system and departmental policies and procedures

Organizes Orientation for new staff

Responsible for balancing the clinical schedule

Maintains active and current professional credentials

Participates in interviewing, hiring, disciplinary process, firing with the Program Director

Experience Required

3-5 years

Education Requirements

Bachelor’s Degree

Master's Preferred

License/Certification Requirements

New Jersey Registered Nurse
