Lead Options Counselor – social service support coordination
Due to organizational growth, Liberty Healthcare is hiring for a new full-time Lead Options Counselor position. As a Lead Options Counselor, you will be an active contributor with a statewide Aging and Disability Resource Center system which supports older individuals and people who are living with disabilities. Program objectives include: + Functioning as the “go to” resource for people who are seeking information on long term care and other community-based services and supports + Providing person-centered planning in a way that supports informed decision making and the ability to secure services which will enable people to live as independently as possible Your day-to-day responsibilities as a Lead Options Counselor will involve: + Leading a team of six field-based Options Counselors + Ensuring a significant number of Kansans receive high quality options counseling services + Data analysis and reporting + Participating in quality assurance initiatives + Providing guidance and consultation on complex matters + Meeting with participants and their families in-person and virtually + Assessing participant’s needs, providing information, and answering questions + Connecting people with appropriate services and supports + Documenting interactions, services, and relevant information in computer systems This is a hybrid position in which you will work in-person in Liberty Healthcare’s office in Overland Park, travel throughout Kansas, and work from your home office.
