Developing and overseeing activities and programming.
Facilitating the training of all departmental staff members, including all mandatory and recommended Atlantic Health System-sponsored trainings, state and grantee-sponsored trainings, and all other relevant trainings.
Providing day-to-day oversight of counseling program staff including completion of chart reviews, patient interaction observations, case reviews, and skills coaching, ensuring the needs of team and department are continuously addressed.
Setting the agendas for and facilitation of huddles and staff meetings. Ensuring action items are addressed.
In collaboration with management, provide consultative support to the department team around patient management, service delivery, risk management and ethical issues.
Working collaboratively with Program Manager to evaluate program quality and effectiveness, enhance patient experience, and ensuring quality goals are met.
Preparing coverage schedules for in-clinic and venue-based services, ensuring team members are following established protocols, and providing
Representation of the department/organization in internal and external meetings and groups.
Other duties as assigned.
And all responsibilities of Prevention Counselor as required.
Complete comprehensive assessments, including sexual health screenings, to determine program participants’ suitability for evidence-based risk reduction methods, including PrEP initiation and adherence. Perform rapid testing services (including for HIV and HCV) for eligible program participants utilizing approved testing algorithms and NJ State Department of Health approved program policies and procedures. Collect and prepare samples of human fluids for laboratory processing for program participants using approved policies and procedures. Reduce barriers to medical care and interventions by providing linkage to health insurance, clinical providers, pharmacy services, medication and financial assistance for people at high risk for HIV infection. Coordinate wraparound services to support program participants in their risk reduction plans, including transportation, clinical referrals, financial counseling and assistance, housing linkage, mental health and substance use referrals and other supportive services. Regularly monitor program participants to ensure regimen adherence and clinical follow-up visits as per clinical provider assessment and recommendations. Assist and refer clients with prior authorizations, insurance enrollment and medication assistance programs. Work collaboratively with clinical providers both internal and external to the Atlantic Health System to ensure appropriate, individualized referrals. Utilize data entry software and electronic medical records to ensure accurate and HIPAA compliant record keeping. Provide risk reduction and treatment adherence counseling as appropriate. Compose comprehensive progress notes using a data, assessment, plan model. Participate in local public health planning initiatives in line with the Ending the Epidemic strategies. Engage in community outreach, including collaboration with social media platforms and dating applications and attendance at local health fairs and events. Monitor and evaluate program and affiliated grants with the support of management. Collect program data and compile and report program results/indicators. Provide all services in the clinic as well as travel to venue-based sites to provide services in alternative settings including homeless shelters, drop-in centers, substance use treatment facilities, colleges, and others.
