The RN Lead for the Healthcare Transformation Program will be responsible for direct oversight of clinical operations for an innovative collaboration between Swedish Hospital and local Federally Qualified Health Centers (FQHC’s). Through this collaboration Swedish Hospital specialty care lines will be embedded in FQHCs to reduce access barriers and address social determinants of health. The Lead RN will be responsible for clinical case management and day-to-day operations of patient care services at the Swedish campus and off-site FQHC locations. The position is supported by Health Care Transformation Program funding of the Illinois Department of Health Care and Family Services (HFS).
What you will need:
Bachelor’s Degree in clinical field, e.g. BSN
Current Registered Nurse licensure required.
Current CPR certification with AHA or Red Cross required.
Minimum five (5) years of experience in increasingly responsible clinical positions including some experience with leading or supervising others
Preferred fluency in Spanish, Arabic, Korean, Vietnamese or another language.
General Summary:
The Lead Registered Nurse is responsible for performing care and assisting the Practice Manager in the day-to-day supervision of Medical Assistants or other clinicians and allied health staff within specific clinical protocols.
Responsibilities and Functions
Participates in the hiring, supervision, and evaluation of clinical assigned staff.
Schedules staffing to meet changing workload needs.
Provides professional and technical support to staff and troubleshoots the delivery of patient care. Responds to requests from families and other concerned patients regarding patient care issues.
Identifies training needs for staff and facilitates staff development. Participates in the development, implementation, and evaluation of training, orientation, and clinical education programs.
Maintains clinic ordering functions for multiple clinic sites
Develops, implements, monitors, and evaluates quality assurance activities, ensuring compliance with standards, laws, regulations, and procedures. Recommends new and/or revised policies and procedures to the Practice Manager.
Reviews and uses clinical and medical records to determine health care patterns and assignment of appropriate resources. Ensures corrective actions.
Collaborate with Healthcare Transformation teams, including Patient Scheduling Representative and CHW, and Community CARE programs. Other projects and duties as assigned
Recognition of the physical exam process and the ability to record the exam and or procedure findings and details.
Works directly with providers to provide clinical support, which may include prior authorizations, clinical triage, result discussions, and medication management.
