The Patient Progression RN Coordinator acts as a liaison between Patient Placement Center nursing units, Emergency Department, procedure areas, and other patient intake areas to optimize appropriate and timely placement and flow of patients throughout the health system. Utilizes clinical judgment and knowledge of clinical areas and escalation as needed to facilitate decision-making. Prepares and communicates relevant data for dissemination and situational awareness. Supports and provides coverage for department team members as needed. Responsible for the administrative or programmatic management of designated functions for a specialized department/area. The Patient Placement RN will act as a liaison between internal and external physicians, nursing, ED, PACU, and other patient intake areas to expedite the appropriate and timely placement of patients. Coordinates and facilitates the placement of patients using clinical nursing skills and knowledge of clinical care requirements. Maintains knowledge and practice of outside hospital transfer facilitation and barrier mitigation in coordination with the Transfer Center.
Provides oversight for the Health System and activates the appropriate clinical operational or logistical resource(s) in collaboration with department leadership and leaders across the organization.Coordinates the efforts and functions within the department for daily operations, provides ongoing advice and consultation to the staff both internal and external, problem solves capacity and patient flow barriers and obstacles, coordinates efforts to facilitate flow and priorities based on institutional needs, and evaluates appropriateness and effectiveness of daily functions and strategies Manage deviations from patient care pathways; resolve bottlenecks, variation in service level agreements, optimize communication and prioritization with support services and a variety of clinical departmentsCollaborate with units and all other pertinent departments to identify process improvement strategies, identify and analyze trends causing delays, support quality initiatives and enhance patient safetyDemonstrate positive, effective communication skills and professional behavior to promote cooperation and teamwork to enhance patient flow.Exhibit leadership and collaborates with the interdisciplinary team to improve and sustain the quality and effectiveness of services to patients in an interdisciplinary setting.Train to and function in the front line role (bed center and transfer center) when needed to facilitate appropriate placement and transfers of patients based on level of care and medical necessity in accordance with policy and procedure. Lead RN will rotate through frontline shifts regularly to maintain proficiency and competency. Maintains ongoing scan of admissions, ED activity, OR and PACU activity.Communicates with the emergency department, PACU, and inpatient units frequently to prevent back up of admissions.Reviews patient flow data for upcoming one to two weeks and plans for anticipated census in collaboration with unit leadership. Includes reviewing OR schedule for following day and upcoming seven days.Assist with departmental call-out needs as ableAssist department leadership with routine quality audits and case review requestsIn partnership with the Transfer Center, Triage MD and Admitting Department, monitors bed availability on all units including ICU, IMU, and acute. MINIMUM REQUIREMENTS
Education: Graduate of an accredited nursing program required. Bachelor’s of Science Nursing Degree is required within 5 years of hire or transfer into this position.
Experience: 3 years relevant experience.
Licensure: Licensed to Practice as a Registered Nurse in the
Commonwealth of. Virginia required. Valid Virginia Driver’s License (VADL) may be required.
