Lead Social Worker

Job Summary

Provides social, financial, and psychological support and problem resolution to inpatients, outpatients and their families related to their health condition spanning the entire heal care continuum.

 

Responsibilities

• Receives and responds to referrals from case managers, physicians, nursing, family, extenders, nurse managers, administration, ancillary services to coordinate and implement the discharge plan as needed. • Coordinate out of home placements with patient/family/and other facilities. • Assist patients and families in understanding and making constructive use of patient care services and community resources (lodging, parking, meals, transportation, prescriptions, government entitlement program referrals, etc.). • Identify socioeconomic conditions relevant to the health status of patients and stays current in knowledge of various program eligibility guidelines. • Complete required psycho-social assessments for patients upon admissions. • Assist patients and families with psycho-social adjustment to illness or injury, including: crisis intervention, bereavement issues regarding loss, psycho-social support, support groups, establish treatment plans, etc. • Respond to referrals for patients and families with complex issues and needs including: Guardianship Issues Adult abuse/neglect Child abuse/neglect Sexual assault/rape Domestic Violence Adoptions Out of home placements (i.e. ICF, SNF, PCH, Rehabs, etc.) Advance directives Diversion of social admissions Veterans • Respond to educational needs of the staff and community, specifically related to social work issues such as advance directives, teen pregnancy, abuse/neglect issues, etc. • Education to patient/family/community on advance directives. • Assistance to patient and family with completion of advance directives, organ donation, etc. • Participate on hospital committees as needed, that are necessary and appropriate such as Ethics, Quality Improvement Councils, Medical Audit, Patient Care, etc. • Assist in facilitation of communication to patient, family, police, employer, and others as necessary and appropriate. • Works as an integral part of the interdisciplinary team of assigned area. • Document all action done for or with patient/family in Medical Records.

 

Knowledge, Skills & Abilities

Patient Group Knowledge (Only applies to positions with direct patient contact) The employee must possess/obtain (by the end of the orientation period) and demonstrate the knowledge and skills necessary to provide developmentally appropriate assessment, treatment or care as defined by the department’s identified patient ages. Specifically the employee must be able to demonstrate competency in: 1) ability to obtain and interpret information in terms of patient needs; 2) knowledge of growth and development; and 3) understanding of the range of treatment needed by the patients. Competency Statement Must demonstrate competency through an initial orientation and ongoing competency validation to independently perform tasks and additional duties as specified in the job description and the unit/department specific competency checklist. Perform work in accordance with all departmental pay practices and scheduling policies, including but not limited to, overtime, various shift work, and on-call situations. 7. Perform work in alignment with the overall mission and strategic plan of the organization. 8. Follow organizational and departmental policies and procedures, as applicable. 9. Perform related duties as assigned.

 

Education 

• Master's Degree (Required) Education: Masters Degree - Social Work or Education or Psychology or Counseling or Sociology Experience: 1 Year - Related Experience Substitution: Bachelor Degree in Social Work + 5-7 Years Experience May be Substituted for MSW and 1 Year Related Experience.

 

Credentials 

• Licensed Social Worker (Required)
