Lead SW Care Coordinator II
Overview M Health Fairview has an immediate opening for a Lead Social Worker Care Coordinator II to support the Fairview Partners (FVP) team. This position will serve Fairview Partners members in the south Twin Cities metro and surrounding suburbs. This is a 1.0 FTE (80 hours per two week pay period) opening. This position will serve Fairview Partners members in the Twin Cities metro area - specifically Minneapolis and surrounding southwest suburbs. Responsibilities Job Description The Lead Fairview Partners (FVP) Social Work (SW) Care Coordinator (CC) - II performs all the job expectations of the FVP SW Care Coordinator and acts as a lead resource for best practice on a regional care coordination team. In partnership with a FVP CC Manager, the Lead CC will reinforce policies and procedures, provide case consultation and coverage to colleagues, and assist with departmental projects as assigned. This lead position role models the Fairview Commitments by supporting process improvement, communicating effectively with members and interdisciplinary colleagues, and providing exceptional customer experience. The Lead SW CC - II carries a reduced caseload to allow for these additional duties (described below), as assigned, but is still actively providing care coordination per department, FV system, state, and federal guidelines. Job Expectations: Be a regional team resource for care coordination best practice on new and established care coordination workflows, policies and procedures including, but not limited to: MMIS, MnChoices, health plan requirements, and state and federal regulations. Be available to colleagues for case consults and be a ‘first call for help’ for troubleshooting individual CC questions. Be a clinical practice resource to other social workers in the care management department. Participate in interdisciplinary team meetings, as requested. Assist CC Manager team with chart reviews and monthly audits, as assigned. Assist regional CC Manager in reinforcing the Daily Engagement System by hosting huddle in manager’s absence and being a champion for continuous improvement projects on the team. Assist with or lead special projects and data gathering for health plan requests/reports. Provide care coordination coverage during departmental staffing shortages due to employee leaves of absence, resignations and terminations. Cover caseloads and/or assessments, as needed, for colleagues who are training new employees. Contribute to workflow design including, but not limited to electronic medical record (Epic) documentation, support tools and best practice guidelines, as requested by leadership. Represent Fairview Partners front-line staff to the system, when delegated by leadership. Participate in interview process for potential employees, when requested by department leadership. Attend health plan care coordination meetings with CC leadership team. Present to colleagues at care coordination staff meetings or team huddles, as needed. Model best practice in social work care coordination and case management. Model the Fairview Commitments of Respect for People and Continuous Improvement. Fosters a culture of improvement, efficiency, and innovative thinking. Performs other duties as assigned. Qualifications Required Education Bachelor’s degree in social work or related field. Experience Three to five years’ experience in medical social work, care coordination with managed care, state, and public programs and/or health care systems. Critical thinking and ability to work with patients with complex health and psychosocial issues a must. License/Certification/Registration Current Minnesota Social Work license in good standing Current Driver’s License in good standing Current Basic Life Support (BLS) for Health Care Provider from American Heart Association (AHA) or American Red Cross Knowledge/Skills: Working knowledge of electronic medical record systems and Minnesota Medicaid Information System. Proficiency with MS Office programs, excellent communication, and presentation skills (written and verbal). Strong knowledge base of case management/care coordination principles and best practice. Preferred Education Bachelor’s degree or higher in social work or related field. Experience Five to seven years care coordination experience with managed care, state, and public programs and/or health care systems. Experience leading projects, workgroups, or committee in a workplace setting. Strong technology skills, especially with Microsoft Office programs, web-based systems, and electronic medical records.
