Responsible for providing designated mental health services to child, adolescent, adult and geriatric age groups on an outpatient basis. Under the direct supervision of the Clinical Manager, is responsible for mental health outpatient services provided to a caseload of clients for a designated shift, 40 hours per week working late nights if needed to best serve the client. Employee subject to overtime and call back as required.ESSENTIAL JOB FUNCTIONS:
Provides client and family education, support, and care in gender-, spiritually-, economically- and culturally-responsive, trauma-informed, humble, kind, and competent waysFollows HIPAA guidelines and maintains the highest level of confidentiality and privacy at all times when dealing with client informationProvides therapeutic services to assigned caseload in ways that seek to promote individual, program and system-level approaches that foster health and resilience, increase necessary supports while reducing gaps in service and barriers to social inclusionProvides brief individual, family, group and marital counseling with children, adolescents, adults, families, and couples with the provision of collateral counseling as neededFacilitates equitable services for recipients in person and via telehealth and telephonic options as needed, safe, appropriate, and beneficialCompletes intakes and makes referrals any additional services needed including but not limited to Agency-based individual therapy, medication evaluation and management, group therapy, and case managementProvides coordination and follow-up monitoring of services rendered to the client and family as neededMaintains or exceeds productivity standardsDevelops and/or maintains a Treatment Plan related to clients’ presenting problems and utilizing objectives and interventions that supports client strengths, goals, and choiceCompletes and/or ensures the completion of all appropriate documentation in a timely manner with correct and appropriate tone, grammar, and syntax. This includes but is not limited to Progress Notes, Informed Consents & Permission to Treat, Orientation Acknowledgement, Medical History, Assessments, etc.Collaborates effectively with clients, outside agency personnel, family members, and internal agency personnel including but not limited to supervisors, therapists, physicians, nurses, and case managers to ensure continuity of careProvides crisis assistance in situations when immediate attention is warrantedProvides community education, awareness, and support regarding of mental health and other relevant issues as ableProvides reporting of work activity to Clinical Manager as requestedParticipates in staffing and supervision as scheduled and as needed, and displays evidence of the value of lifelong learning as related to best practices in client careParticipates in Quality Assurance & Improvement, Utilization Management, and other agency and system functionsOther activities or responsibilities as needed per supervisor or agencyJOB SPECIFICATIONS:
EDUCATION:
Master’s Degree requiredLICENSURE, REGISTRATION, CERTIFICATION:
Maintains an active license with the State of Tennessee Health Related Boards as a Licensed Clinical Social WorkerAny additional certification or training as requested by supervisor or AgencyEXPERIENCE:

Able to perform various physical demands of the job that help deliver services to program recipients and the agency.
