Function as an active member of the multi-disciplinary team in planning appropriate treatment and continuing care plans for patients. Provide psycho-social evaluations, has knowledge of psychosocial issues affecting the families of seriously ill neonates/infants and patients, respond to referrals, screen all high-risk patients, teenage mothers, and suicidal/homicidal patients for discharge planning and referrals. Assist the patient/family in choosing/utilizing appropriate services.

QualificationsLicense required (LCSW) Graduate from an accredited school with a Masters Degree in Social Work or related field including Psychology, Sociology, or Marriage and Family Therapy. One year experience in the acute care setting is preferred.
