Qualifications

Experience working with the geriatric population (preferred) Must meet credentialing standards of employer and CMS Willing to travel to assigned Nursing Homes within the local area Experience collaborating as part of a multidisciplinary team Basic knowledge of Microsoft Office, and experience using Electronic Medical Records Systems Must possess effective communication skills in order to interact with patients, families, and facility staff
