POSITION SUMMARY

This position is responsible for providing social services support as requested by the physicians and other Medical staff and to perform counseling and demonstrate the ability to provide positive and supportive communications to patients and families.  Key responsibility is to facilitate the smooth transition of patients from one level of care to another using available resources.  Participation in the multi-disciplinary team meetings is another responsibility of this position.

QUALIFICATIONSEducation:  Bachelor’s degree in Social Work, required.Experience:  One year of experience in a related field required. Hospital and/or Long Term Care experience preferred.BLS required PHYSICAL REQUIREMENTSEye and hand coordination, manual dexterity for computer and writing skills and visual acuity required.  Ability to communicate using written and verbal media.  Ability to spend extended periods of time sitting and walking, bending and stooping may be required.EEOC Statement“Harris Regional Hospital is an Equal Opportunity Employer.
