Licensed Clinical Social Worker - Accredo - Remote

Responsibilities:

Telephonic outreach and customer engagement, support and assistanceSupport and coordination of complex situations - leveraging community resourcesEducate and refer customers to available social and health resourcesAssist in the creation, execution, and management of the pulmonary arterial hypertension (PAH) licensed social worker program, implementing and adjusting the program as needed to best meet the needs of the patient populationConsultation with Cigna Medical Management teams and Cigna Behavioral Health teams and programs as appropriateIdentifies and educates appropriate client specific programsEmpowers customers with skills to provide enhanced interaction with their providersDocuments customer interactions and interventions as directedConsult with and assist team members in cases where a customer’s medical and behavioral issues are impacting their ability to set and/or achieve goalsCollaboration with team members such as nutritionist, pharmacist, case management nurses, specialty team member, Medical Director and other internal partners across Cigna's programs as appropriateProvide ongoing consultation and training to the team on relevant social topics, resources availableProvide connection and assistance to agencies where alternative funding may be needed (i.e. SSI, SSDI benefits, state victim assistance, VA state funded rehab or community resourcesLife planning needs such as end of life, home/financial management and accessible housingResources for community resources for conditions and bereavementAssumes lead role with customer when appropriate but integrates with warm connection with Cigna team members to support ongoing care and needsShift: Monday-Friday; 9AM-5:30PM EST / 10AM-6:30PM MST / 11AM-7:30PM PST

Qualifications:

Master’s DegreeLicensed Clinical Social Worker (LCSW)Minimum of five (5) years clinical experience in behavioral health/social work fieldExperience with crisis intervention and psychosocial assessmentsExperience in health education and wellness coaching preferredExperience with direct customer communication (written and verbal), experience in telephonic counseling/coaching requiredWorking knowledge of motivational strategies, training and consultation requiredThorough knowledge and understanding of health behavior change theories and their applicationProven administrative abilities, with strong computer and software application skillsExcellent interpersonal skills and the ability to work in a team environmentDemonstrated ability to set prioritiesA high energy level and excellent written and oral communication skills are essentialPassion for health improvement of customersPreferred CompetenciesExcellent time management, organizational, research, analytical, negotiation, communication (oral and written) and interpersonal skillsStrong personal computer skills, MS word, Excel, Outlook experience, and Internet research desiredExperience creating and managing social work programs a plusKnowledge of the insurance industry and claims processingService marketing/sales and business acumen experience a plusStrong skills in the following areas: teamwork, conflict management, assessment complex issues, ability to recommend changes and resolve problems through effective decision makingExperience in medical management and case management in a managed care setting is highly desirable  Knowledge of managed care products and strategiesDemonstrated sensitivity to culturally diverse situations, customers and clientsAbility to work within changing business environment and balance business needs with patient advocacyExperience managing multiple projects in a fast-paced matrix driven environmentDemonstrated ability to coach and educate other case management staffDemonstrate effective organizational skills and flexibility to meet the business needs

For this position, we anticipate offering an annual salary of 56,600 - 94,400 USD / yearly, depending on relevant factors, including experience and geographic location.

This role is also anticipated to be eligible to participate in an annual bonus plan.
