Job Description​

The Social Worker LCSW or QMHP in the Emergency Room is responsible and accountable for assessing the psychological social emotional cultural and or financial needs that impact patients health and recovery. To provide professional social work services that utilize supportive counseling and clinical knowledge in assessing the needs of patients and families during illness crisis intervention and post discharge planning. Collaborates with members of health care team in the development of a transition plan and addresses needs in a timely manner. Participate in program planning and development to meet the needs of customers within the health care system and community.

 

The Social Work team in the ED are being consulted on many patients with a positive mental health history in order to provide needed mental and behavioral health services and follow up, in addition to the consults requesting assistance with the three social determinants of health identified by the ED triage process.

 

PRN staff must be available to work overnight and weekend shifts. Requirements include Missouri Social Work License (LMSW - QMHP or LCSW), Masters Degree in Social Work and experience in assessments of mental and behavioral health issues.
