Coordinates, implements, and arranges for post-discharge extended care, home health and durable medical equipment for patients in collaboration with Care Managers, patients/families, physicians, caregivers, staff, and community agencies. Consults, collaborates and coordinates plan of care for patients with physicians and providers with a focus on behavioral health clients. Completes psychosocial assessments, and provides crisis intervention, and emotional support with regard to diagnosis, treatment, and post-discharge plan as indicated. Identifies and accesses resources to optimize effective and beneficial patient care outcomes in collaboration with a multi-disciplinary team. Collaborates with a multi-disciplinary team to provide care and resources to patients in departments across the organization as well as Cone Behavioral Health (CBH). Master’s Degree from an accredited School of Social Work required. LCSW (Licensed Clinical Social Worker) required. Excellent communication skills required, with an emphasis on oral and listening abilities with both patients and staff members. Possess individual therapy skills for all patients, analytical thinking, problem-solving aptitude and ability to set priorities. Knowledge of treatment modalities and alternate healthcare delivery systems is preferred.Minimum one year experience working in a Behavioral Health organization or with Behavioral Health Clients required. Knowledge of social and health care resources within the community. Familiarity with standards for medical social work practice Two years’ experience in a health care-related field preferred.
