Summary/Objective
Overseen by Clinic Management, the licensed therapist (bilingual), is responsible for providing ongoing therapy, assessment and case management services to individuals admitted to the clinic. The licensed therapist is a motivated and organized individual who can manage the varying needs of his/her caseload. The licensed therapist will ensure a therapeutic and supportive milieu with the individuals on their caseload.
Duties and Responsibilities
Provide therapy and other related services including but not limited to initial assessments, treatment planning, routine case management and crisis services as needed
Ensure documentation meets quality and compliance thresholds as set by regulation and/or Agency guidelines
Utilize electronic medical record to complete required documentation as required by regulation and within mandated time frames
Prevent, recognize and report abuse toward the people we support
Assess and diagnose individuals as needed
Adhere to the organizations Code of Ethics
Always communicate in a respectful manner with all people including the people we support, supervisors, other valued team members, and members of the community
Complete all required trainings
Maintain confidentiality
Take part in new treatment initiatives including but not limited to, integrated care, smoking cessation treatment and co-occurring disorders treatment
Maintain and complete responsibilities of client caseload including but not limited, treatment plans, progress notes and assessments
Assist the Clinic Director in reporting activities including but not limited to Pscykes, OMH, Financial and County reporting requirements
Act as part of an interdisciplinary team to ensure a coordinated, comprehensive and holistic approach to the provision of services
Effectively communicate and support the organization’s mission, vision and values
Other duties as assigned
Education, Work Experience, and Competencies
Licensed therapist (i.e. LMHC, LMSW, LCSW, LCAT)
At least 1 year experience providing individual, group or family therapy to individuals with mental health/substance use barriers.
Valid driver’s license with no more than 8 points
No convictions for crimes that preclude a person from working with members of a vulnerable population
License actively registered and current with the Office of Professions
Active NPI with Medicaid and Medicare(if LCSW) numbers/privileges
