Under supervision of Program Director, the Social Worker is responsible for the provision of short- and long-term psychotherapy to a caseload of psychiatrically diagnosed individuals. The clinician is key at assessing, screening, and connecting individuals with appropriate levels of care. This position requires great organizational and. time management skills. The salary is $65,000/annually KEY ESSENTIAL FUNCTIONS: Carries an active caseload of clients as stipulated by the clinic.Provides 28 hours of direct care services each week.Facilitate groups weekly.Engages significant other and/or family members in treatment exceptions are documented in the case record.Completes all required FIT training modules.Responsible for making sure the case record always remains up to date.Submit required clinical documentation adhering to OASAS regulatory timeframes.Participates in clinical supervision as scheduled.Participates in bi-weekly staff meeting.Partners with front desk team to ensure all clients are scheduled.Conducts daily outreach calls.Consults with the clinical supervisor all cases ready for discharge and submits all discharge related paperwork as scheduled.Completes PHQ-9 at admission and eve1y 90 days.Conducts audits of charts and submit monthly.Utilizes collaborative documentation with 90% of clients.Submit daily progress notes. Engages all clients in the creating of a treatment plan as evidenced by submitting plans with all signatures.Other duties and responsibilities assigned by management.
