Summary of Job Duties:We are seeking a qualified Licensed Master Social Worker (LMSW) to join our dynamic team as a Primary Care Behavioral Health Faculty (PCBHF). As an integral part of our patient-centered medical home, the PCBHF will provide direct care to clinic patients, as referred by physicians, while working closely with the treatment team to promote patient health outcomes. Additionally, the PCBHF will be responsible for the development and provision of behavioral health curriculum for family medicine residents, as well as collaborating with other clinical team members and health students.

Key Responsibilities:Direct Care: Provide direct care to clinic patients, as referred by physicians, utilizing behaviorist/consultant skills. Collaborate with the treatment team to create and implement individualized treatment plans.Health Outcomes Promotion: Collaborate with the treatment team to promote patient health outcomes, working closely with primary care physicians to address physical health and behavioral health needs.Behavioral Health Curriculum Development: Develop and revise behavioral health curriculum for family medicine residents, ensuring content is up-to-date and aligns with current best practices.Collaboration: Work closely with clinical team members and health students to provide education, consultation, and support in incorporating behavioral health into primary care practice.Patient Education: Educate patients about behavioral health issues, treatment options, and the importance of regular mental health check-ups.Evaluation: Conduct ongoing evaluations and assessments to evaluate the effectiveness of treatment plans and make necessary adjustments.Professional Development: Stay updated with the latest advancements in behavioral health and attend relevant conferences and trainings.Administrative Duties: Complete administrative tasks, such as documentation and reporting, as assigned.Other Duties as assigned.
