About the Position The Physical Therapist evaluates patients, under physician referral, with decline in ability to perform functional tasks, such as gait and transfers, impaired safety level, pain, and impaired quality of life due to illness, injury, and disease processes. Develops effective treatment plans and goals, obtaining approval for services from the referring physician. Involves the patient and caregivers in the development of the Plan of Care. Communicates and teaches families and other health team members to improve outcomes and follow-through with exercise and gait programs and functional tasks. All tasks are performed in a home health setting, with assessment of eligibility of homebound status performed each visit. Job Duties + Provide comprehensive Physical Therapy evaluations and treatments, under referral from the attending physician, to overcome specific losses of function, impaired safety, and pain. + Open cases, completing Oasis assessment, if on intake the patient is assessed as having no nursing needs. + Evaluate the home environment and make appropriate recommendations. + Consult with DON and nursing staff, after Oasis assessment and Physical Therapy evaluation, on needs for additional services, such as nursing, home health aide, occupational therapy, or speech therapy. + Effectively communicate with the referring physician and nursing staff concerning changes in the patient’s condition, and necessary changes to the initial Plan of Care due to such changes. + Coordinate treatment with nursing staff, home health aides, occupational therapists, and speech therapists, as necessary. + Document evaluations, treatment sessions and services provided in the home according to agency protocols and Medicare guidelines, completing all documentation in a timely and effective manner for both communication with other health professionals and billing purposes. + Effectively involve the patient and caregivers in the development of the Plan of Care, and any changes indicated to the Plan of Care due to a change in condition. + Provide education to patient and caregivers through effective teaching methods in home exercises, safety with functional activities, and carryover of other therapeutic techniques as appropriate. + Provide supervision of Licensed Physical Therapy Assistants (LPTAs) involved in the patient’s care according to guidelines for supervision established by the Commonwealth of Virginia state practice act and Medicare guidelines. + Participate in teaching of LPTAs through required joint visits and as needed in patient specific procedures and treatment techniques. + Maintains privacy and confidentiality of patient information at all times. + Implement infection control measures per agency protocols and policies. + Use equipment and supplies effectively and efficiently. + Participate in case conferences and discharge planning per agency protocols. + Complete Oasis recertification and discharge assessments if indicated. + Attend at least 4 staff meetings per year. + Attend agency mandatory in-services and maintains 100% compliance with mandatory data based training. + Participate in implementing systems for evaluating the home health agency’s delivery of services to maintain the highest quality of patient care and satisfaction with services possible. Job Requirements + Degree in Physical Therapy from an APTA approved and accredited college or university. + Current licensure as a Physical Therapist in the Commonwealth of Virginia. + CPR certification (adult only) + Passes an assessment of competencies prior to providing patient care, with any unmet areas addressed through education and mentoring prior to participation in patient evaluations and care. + Possesses the ability to deal tactfully and compassionately with patients, family members, and caregivers. + Has the ability to motivate patients to participate as fully as possible in their rehabilitation programs. + Possesses the ability to make independent decisions. + Professional dress, demeanor, and presentation. + Has the ability to move intermittently throughout the workday, lift, push, pull, and move equipment, supplies, etc. up to 50 lbs. getting assistance when needed is a must. + Possesses enthusiasm, flexibility, and the ability to adapt to the changing healthcare work environment. + Possess a Virginia Driver’s license, proof of automobile insurance and readily available, reliable transportation.
