Licensed Practical Nurse
Position: Licensed Practical NurseLocation: Saluja Medical Associates 
The LPN is responsible for supporting patient care in a primary care concierge medical practice with professional service, excellent patient care, and engaged support to both the clinical and management teams. ACO experience is preferred.ESSENTIAL ROLE OBJECTIVES:Support the physicians by providing clinical and administrative assistanceProvide physical care and carry out therapeutic and medical regimens.Obtain patient histories and medical informationTriage and assess client health concerns to determine appropriate responseAssess and report vital signs and measurements; collect specimensAdminister medications and treatment as ordered by physicianAssist with medical and minor surgical proceduresPrepare patients for continuing care after treatment.Treat and dress woundsProvide patient education for scheduled procedures and self administration of medicationSchedule and keep track of appointmentsTrack and document patient status following sick visitsObtain authorization for prescriptions as required by insurance carriersManage electronic and faxed prescription refill requestsScanning documents to patient chartsMaintain examination room inventory and replenish as neededUphold appropriate clinical standardsMaintain safe and clean working environment by implementing and monitoring procedures, rules, and regulationsManaging and supervising  medical assistants in the office including scheduled  inservice evaluations. Customer service champion Responsible for value based initiatives including patient outreach, annual wellness visits, transition of care, and gaps in care.Assist with hiring and training of new clinical staff (Medical Assistants) JOB QUALIFICATIONS & COMPETENCIES:Maintains a current LPN/RN license in the state of Maryland At least two years of experience in primary care settingAble to work with a team of professionals involved in providing exceptional customer service and outstanding health careComputer literacy, including MS Office and electronic medical records systemsSalary: $60,000-$75,000 based on experience
REPORTING AND OTHER EXPECTATIONS:The position will report to the Practice Manager for day-to-day supervision. The person in this position will also have significant contact and involvement with other members of the clinical team.
