As an LPN, you'll be at the forefront of providing vital nursing services in outpatient psychiatry, addiction recovery, and housing programs. Key Responsibilities:
· Administer medications to clients using various methods, including oral, injection, and self-administration, adhering to service line protocols.
· Conduct thorough nursing assessments, including physical and mental health evaluations and psychiatric screenings.
· Maintain accurate and up-to-date documentation of medication administration and nursing assessments.
· Oversee medication counts, manage house stock medications, and ensure timely refills for housing clients.
· Provide client education on medications and stage refill requests to providers per standard operating procedures.
· Facilitate client intake by collecting preliminary data, including medical history, vital signs, and lab samples.
· Collaborate with clients to develop safety plans and provide nursing-led educational groups.
· Ensure compliance with infection control standards and address hygiene and health-related issues.
· Act as a resource for nursing and medical concerns, supporting overall client and facility health under nursing leadership.

Education, Experience, and/or Credential Qualifications:
· Current licensure as a Licensed Practical Nurse (LPN) in the state of practice.
· Graduation from an accredited practical nurse program.

Additional Qualifications:
· Current driver’s license, acceptable driving record, and auto insurance.
· Strong communication and problem-solving skills with a commitment to confidentiality and professionalism.
· Proficiency in electronic health records and the ability to interpret lab results and procedural documents.

Physical Requirements:
· Perform sedentary work involving occasional lifting or carrying of up to 10 pounds.
· Frequent repetitive hand and arm movements for typing and writing during shifts.
· May involve brief periods of walking or standing.
