Job Summary:
The Licensed Practical Nurse (LPN) is responsible for providing nursing services to patients within residential care facilities, behavioral health settings, and/or substance use disorder treatment programs. The role includes maintaining a safe and therapeutic environment for clients while performing nursing duties across outpatient psychiatry, addiction services, and housing services. The LPN will also work to build and maintain strong relationships between clients and clinic staff to ensure quality care and a positive client experience.

Key Responsibilities:
Medication Administration: Administer medications to clients using various methods, including oral, injection, and self-administration, in accordance with service line requirements.
Nursing Assessments: Perform nursing assessments, including physical and mental health evaluations, psychiatric screeners, and other health assessments as required.
Documentation: Accurately document medication administration, nursing assessments, and client progress, ensuring all records are up-to-date and meet service line standards.
Medication Management: Conduct medication administration record (MAR) reviews, medication counts, and monitor house stock of medications for housing clients, including preparation of pass medications as needed.
Client Communication: Communicate with clients regarding their medications, answer medication-related questions, and stage refill requests to providers according to standard operating policies and procedures.
Client Rooming: Room clients by verifying client data and medical history, confirming the purpose of their visit, and obtaining necessary preliminary requirements such as blood draws, urine samples, vital signs (blood pressure, pulse, temperature), and other assessments.
Safety Plans & Interventions: Complete required documentation promptly, including nursing assessments, interventions, safety plans, and nursing-led educational groups.
General Medical Support: Assist with general lab work, phlebotomy duties, and other clinical tasks as needed.
Resource for Health Issues: Function as a resource for nursing and medical issues, overseeing infectious disease control, client hygiene, and other health-related issues within the facility under the supervision of nursing leadership.
Team Collaboration: Participate in required meetings and communicate effectively with team members to ensure holistic care for clients. Perform other related duties as assigned or delegated by nursing leadership.

Documentation Skills: Proficient in entering, transcribing, recording, and storing information in both written and electronic formats.
Program Evaluation: Able to assess and provide feedback to improve program and service effectiveness.
Communication: Strong verbal and written communication skills, with the ability to produce clear, concise reports and convey complex information in a respectful manner.
Confidentiality: Committed to maintaining confidentiality and safeguarding sensitive information, including medical records, through secure methods.
Interpretation Skills: Adept at interpreting laboratory results, health records, and safety guidelines.
Mathematical Competency: Competent in basic mathematical calculations, such as addition, subtraction, multiplication, and division of whole numbers, fractions, and decimals.
Problem-Solving Skills: Strong ability to understand and follow instructions while adapting to new situations.
Willingness to Learn: Demonstrates a willingness to learn, grow professionally, and work collaboratively with others in a diverse environment.
Experience and Education Qualifications:
Licensure: Current licensure as a Licensed Practical Nurse (LPN) in the state of practice.
Education: Graduation from an accredited practical nurse program.

Employment Requirements:
Background Check: Successful completion of background checks, including criminal record, driving record, abuse/neglect, and fingerprint checks.
Orientation & Training: Completion of New Hire Orientation and all required training, including Relias courses, at the beginning of employment and annually thereafter.
Driver’s License: Current valid driver’s license, acceptable driving record, and current auto insurance.
Health Clearance: Must provide evidence of being free from infectious and contagious diseases (such as tuberculosis) prior to employment and annually thereafter.
Physical Requirements:
Sedentary Work: Exerting up to 10 pounds of force occasionally and/or a negligible amount of force frequently (up to 2/3 of the time) to lift, carry, push, or pull objects.
Repetitive Movements: Requires repetitive hand movements (typing, writing) during work shifts.
Physical Activity: Primarily sedentary work with occasional walking, standing, or traveling between work sites.
Client Transportation: Ability to transport clients when necessary.
