We are looking for enthusiastic and compassionate Licensed Practical Nurses (LPNs) to join our dedicated team at the Lower Fairfield Center (LFC) in Norwalk, CT. + Provide direct nursing care to individuals with intellectual/developmental disabilities and/or autism spectrum disorder.
+ Coordinate medications and treatments as prescribed by healthcare providers.
+ Document all care provided in individuals' medical records and complete required facility documentation.
+ Offer incidental teaching to individuals served, staff, and families concerning medications and their use.
+ Communicate effectively with team members and community providers both over the telephone and in written correspondence.
+ Support individuals who may exhibit mild to severe challenging behaviors.
+ Perform related residential duties as required to ensure the health and safety of individuals.
+ Provide care using basic nursing skills, including but not limited to: coordinating medication and nutrition via G-Tube or J-Tube, glucose monitoring, insulin administration, respiratory treatments, colostomy, cecostomy, urostomy care, wound care, and catheterization.
Work Schedule We offer full-time opportunities across various shifts to meet the demands of our facility. Flexibility in scheduling is required. The available shifts are:
+ 1st Shift (PCN 94499), 70 hours biweekly:
+ Week 1 (32 hrs): Friday 6a-2:30p; Tuesday 6a-2:30p; Wednesday 6a-2:30p; Thursday 6a-2:30p (Regular Days off Saturday, Sunday, and Monday)
+ Week 2 (38 hrs): Saturday 6a-2:30p; Sunday 6a-2:30p; Monday 6a-2:30p; Tuesday 6a-12:30p; Thursday 6a-2:30p (Regular Days off Friday and Wednesday)
+ 2nd Shift (PCN 19868), 70 hours biweekly:
+ Week 1 (35 hrs) : Friday 2p-9:30p; Saturday 2p-9:30p; Sunday 2p-9:30p; Wednesday 2p-9:30p; Thursday 2p-9:30p (Regular Days off Monday and Tuesday)
+ Week 2 (35 hrs) : Saturday 2p-9:30p; Sunday 2p-9:30p; Monday 2p-9:30p; Tuesday 2p-9:30p; Thursday 2p-9:30p (Regular Days off Friday and Wednesday)
+ 3rd Shift (PCN 18348), 70 hours biweekly:
+ Week 1 ( 38 hrs) : Saturday 10p-6:30a; Sunday 10p-6:30a; Monday 10p-6:30a; Wednesday 12a-6:30a; Thursday 10p-6:30a (Regular Days off Friday and Tuesday)

+ RECRUITMENT AND RETENTION BONUS FOR LICENSED PRACTICAL NURSES. Each Licensed Practical Nurse who is permanently assigned to second and third shift shall receive a yearly “retention” bonus of six hundred ($600). This bonus shall be paid in two (2) installments: In July and December. In order to be eligible for this bonus, the Employee must have completed one year of service in the classification.

In a state facility this class is accountable for rendering bedside care or mental health services to individuals with physical disabilities and/or an intellectual disability in instances where the services of a registered nurse are not required.

MINIMUM QUALIFICATIONS
License or temporary permit to practice nursing in Connecticut by the State Board of Examiners for Nursing.
PREFERRED QUALIFICATIONS
+ Experience providing nursing services to individuals with an intellectual/developmental disability and/or autism spectrum disorder.
+ Experience supporting individuals who may exhibit mild to severe challenging behaviors.
+ Experience communicating internally and externally with team members and community providers both over the telephone and in written correspondence.
+ Experience communicating with non-verbal individuals.
+ Experience with basic nursing skills including, but not limited to: administering medication and nutrition via G-Tube or J-Tube; glucose monitoring, insulin administration; respiratory treatments, colostomy; cecostomy; urostomy care; wound care; and catheterization.
+ Experience using Microsoft applications including Outlook; Excel; Teams; and Word.
SPECIAL REQUIREMENTS
DEPARTMENT OF DEVELOPMENTAL SERVICES: Incumbents in this class may be required to possess and retain a valid Connecticut Motor Vehicle Operator's license, Public Passenger Endorsement or Commercial Driver's license for designated positions.
WORKING CONDITIONS
Incumbents in this class may be required to lift/restrain individuals; may have some exposure to communicable/infectious diseases and to some risk of injury from assaultive/abusive individuals; may be exposed to disagreeable conditions.
