Job Description
We are seeking a compassionate and detail-oriented Licensed Practical Nurse (LPN) to join our Primary Care team. This role focuses on providing routine nursing care, high-risk and chronic care management support, and referral specialist support. Our healthcare facility provides primary healthcare, dental, and mental health services to children and adults, with an emphasis on serving the special population living in or near public housing.
ResponsibilitiesProvide routine nursing care to patients.Support high-risk and chronic care management.Assist with referral specialist support.Coordinate care for elderly and disabled patients, as well as patients with chronic conditions such as diabetes, HIV, hypertension, and obesity.Travel between Stamford and Greenwich as needed.Conduct women's health assessments and manage patient vitals.Essential SkillsCurrent LPN licensure in CT.Minimum of 2 years of experience in a primary care or outpatient setting preferred.Experience in high-risk or chronic care management.Knowledge of population health, care coordination, and referral processes.Proficiency with electronic health records (EHR).Excellent communication and interpersonal skills.Ability to work collaboratively within a multidisciplinary team.Bilingual in Spanish or Haitian Creole preferred.Additional Skills & Qualifications3+ years of experience working as an LPN.Experience with chronic care management and comfortable with care coordination.Strong clinical skills in primary care and chronic disease management.High attention to detail and ability to manage multiple responsibilities.Ability to work independently and as part of a team.Strong organizational and time management skills.Compassionate, patient-centered approach to care.Work Environment
You will work in a flexible schedule within an office open Monday to Friday from 8am to 5pm. The community health center sees patients of all ages, from infants to geriatrics. Training can last between 2 to 4 weeks, depending on how quickly the candidate can adapt. Training includes onboarding, orientation, and shadowing. Pay and Benefits
The pay range for this position is $30.00 - $33.00/hr.
