Position Summary: Provides nursing care on an intermittent or continuous care shift basis to patients in their place of residence under the supervision of a Registered Nurse. The LPN is assigned to patients by the Hospice Director and functions under his/her direction.
Education, License & Cert: Graduate of an accredited school of Practical Nursing. LPN currently licensed in Pennsylvania. Knowledge of nursing practice at a level normally associated with a Practical Nurse. High School Diploma or Equivalent required.
Experience: One (1) year nursing experience in an acute care setting and/or previous Hospice experience or similar work with patients/families experiencing terminal illness is preferred. Experience and skill in supervision is desired.
Essential Functions: 1. Provides nursing care in accordance with the standards of the Nurse Practice Act in the commonwealth of Pennsylvania. 2. Assists patients and family in learning self‐care techniques as instituted by the primary nurse. Assists the primary nurse in the performance of specialized procedure for patients assigned. 3. Carries out treatments assigned, utilizing sterile technique and prepares the necessary equipment and materials. Provides direct nursing service after initial assessment by the primary nurse in accordance with Hospice policies. 4. Reinforces appropriate preventative and rehabilitative nursing procedures as outlined by the Hospice Director in the interdisciplinary Plan of Care. Consults with the primary nurse and other disciplines involved in the patient’s care. 5. Prepares clinical and progress notes and documents appropriately according to company policy for all assigned patients. Provides thorough observation of patient status and reports change to Hospice Director or On Call Nurse for appropriate action. 6. Meets the requirements for participation in patient/community/referral source education plan as stipulated by agency policy. 7. May administer sub q and IM medications according to company policy. 8. Attends all in services prior to performing new policies/procedures. 9. Consults RN/Clinical Coordinator regarding any unfamiliar policies/procedures. 10. Carries out schedule/assignment as directed by the Registered Nurse and/or Hospice Director.
