Licensed Practical Nurse (LPN)

Looking for a qualified LPN to join our team! \n

STATUS: non-exempt\/Full-Time\n

SHIFT: 6p-6a (12hr\/3-2 split)\n

Major Responsibilities \n

The Licensed Practical Nurse (LPN) care giver provides direct patient care to the residents and patients of the health and rehabilitation center (skilled nursing home), assisting them in activities of daily living under the direction and supervision of a professional nurse. Responsibilities may include but are not limited to:\n\nAct in the capacity as a Team Leader in a modified primary care setting.\nProvide regular resident status updates to appropriate personnel.\nConduct and document a thorough evaluation of each resident's medical status upon admission and throughout the resident's course of treatment.\nAssist in the implementation of an individualized treatment plan for each assigned resident.\nComplete required documentation in an accurate and timely manner.\nOversee Falls Program and Pressure Sore Program for those residents on their team.\nPerform routine nursing services for residents as directed.\n\n

Specific Requirements\n\nMust be able to read, write, speak, and understand the English language and possess good communication skills.\nMust demonstrate knowledge and skills necessary to provide care appropriate to the age-related needs of the residents served.\nMust possess the ability to make independent decisions when circumstances warrant such action.\nMust possess the ability to deal tactfully with personnel, residents, family members, visitors, government agencies\/personnel, and the general public.\nMust be knowledgeable of nursing and medical practices and procedures, as well as laws, regulations, and guidelines that pertain to nursing care facilities.\nMust be able to relate information concerning a resident's condition.\n\n

Minimum Qualifications\n\nMust possess a current, unencumbered, active state license to practice as an LPN. \nExperience in Skilled Nursing\/Rehabilitation facilities preferred.\n\n
