Schedule: Full Time, 40 hours per week

Shift : Primarily Monday - Friday, 8am to 5pm, rotating weekends

Location : Team Yellow: This position will be conducting patient home visits in the Winter Park, Casselberry, Altamonte Springs, Maitland area which includes but is not limited to the following zip codes: 32701, 32707, 32730, 32751, 32789, 32792

The role you’ll contribute: Provides vocational nursing services under the supervision of an RN, to patients in their homes.

The value you’ll bring to the team:

Performs nursing skills safely and competently.

Performs patient data collection in a holistic manner.

Provides accurate and complete documentation.

Schedules daily visits to facilitate patient needs.

Plans patient care in consultation with Registered nurse.

Assists with patient/caregiver teaching.

Demonstrates flexibility in response to unexpected changes and the ability to adjust priorities and use resources.

Maintains open communication with RN Case Coordinator and RN Supervisor.

Demonstrates suitable time management skills.

Meets agency's expectations regarding productivity.

Participates in monthly case conferences

Participates in agency Performance Improvement activities.

Demonstrates good time management skills.

On call, as requested, RN as back up.

Provides orientation to newly hired LPNs and hosts student LPNs.

The expertise and experiences you’ll need to succeed :

Minimum qualifications :

Licensed Practical Nurse or Licensed Vocational Nurse

BLS Certificate

Valid Driver's License and Auto Insurance

One year of medical/surgical experience

Preferred qualifications:

One year experience in community health nursing

Thirty (30) hours of Florida Board approved IV Therapy Course
