JOB SUMMARY

The Licensed Practical Nurse (LPN) is responsible for the delivery of patient care utilizing the nursing process through data collection and assessment, identifying and determining the priority of patient’s/ resident’s/client’s problems/needs, and evaluating the process and outcome of nursing care.  The LPN coordinates, provides and directs patient teaching activities and coordinates the care provided by health team members.

ESSENTIAL FUNCTIONS

In collaboration with the patient and health team members, conducts initial and ongoing assessment, designed to gather data relevant to the patient’s individual needs.  In an accurate and timely manner, LPN  collects objective and subjective data, modifies the data collected as the patient’s status changes, and reports assessment data as appropriate to other members of the health care team.  Complies to all documentation standards within the scope of the LPN role.

Analyzes the assessment data, evaluates patient's condition, and collaborates with the provider to develop and maintain the patient's plan of care. 

Provides education to patients, families, and staff members, in conjunction with other healthcare entities, under the direction of the provider or manager.

Coordinates the care of the patient with other healthcare entities according to the patient's plan of care.

Administers medications in a safe manner, which is consistent with the organizations policies and procedures and the State of Ohio Nurse Practice Act.

This position will assist the surgeon in surgeries, educate patients on after care/wound care, call patients with results and next step, administrate medications, room patients take patient history, set up rooms for surgery/sterilize rooms prior to surgeries.

LICENSURE/CERTIFICATIONS (must be non-expired/active unless otherwise stated):

Current LPN licensure in the state of the work place.   IV certification may be required for certain specialties.
