This LPN position is responsible for medication management 1 day a week (and additional direct skilled client care if requested) - All under the supervision of a registered nurse. Med Mgmt responsibilities include re-filling client pill boxes, ordering refills, and communicating with Providers and Pharmacists. The clients are in their homes and you would make house calls. Part of the visit would be observing the client and reporting to the RN any changes in condition.

Provide accurate, detailed, and timely documentation consistent with the plan of care and the nursing process Prepare, administer, and appropriately document actions taken specific to the commonly prescribed oral, topical, subcutaneous and intramuscular medications. Observe, report, and record adverse reactions to medications and/or treatment to the registered nurse. Recognize the need for and institute emergency measures in a rapid and appropriate manner. If preferred, perform wound care. Perform all skill, treatments, and procedures to the scope of practice as defined in the Nurse Practice Act in the state of clinical practice and adheres to the policies, procedures, and protocols of the agency Treat clients, family, and staff with courtesy and respectRequirements
Graduated from National League of Nursing accredited practical school of nursing Unencumbered license as a Practical Nurse in the state that care is being provided One (1) year of nursing experience required; three (3) years preferred.
