Deliverables/Principal Results Expected:
Responsibilities for LPN include:
Attend to the nursing issues/records with school students such as Immunization records, height/weight/vision etc. Medication administration responsibilities Maintain accurate records for student charts, updates, parent contacts, doctor contacts, etc. Answer school phones, set up teacher/student/parent/school district meetings for IEPs, etc. Seeks out supervision and direction from physician and program director on completion of nursing assessments- under the direction of the RN, transfers, discharges, treatment plans, progress notes, medication reconciliation, medication administration, vitals, knowledge of detox protocols, assisting medical doctor as needed, provide medical lectures. Report any unusual occurrences to nurse supervisor/program director promptly, ensure room assignments are appropriate to clients’ needs, attend and actively participate in departmental staff meetings. Maintain medical equipment in proper working order; maintain confidentiality and proper boundaries, other duties as assigned.
Responsibilities for Student Support Assistant include:
Assist in keeping the school’s environment safe for students and staff. Assist with proper crisis management techniques and complete appropriate forms. Perform Personal Care Aide duties with toileting and grooming activities when necessary. Assist with activities that are designed by the teacher and/or assistant teacher as part of the daily routine. Communicate to the teacher and/or teacher assistant any issues that arise with the student(s) or within the classroom. Observe student conduct and record behavioral observations on the data collection form as directed. Maintain appropriate boundaries with students and staff. Use a copier, laminator, computer, and drive a van. Monitor students appropriately, never leaving them alone. Assist with any other needs within the classroom. Other duties as assigned.
Technical Competencies: Knowledge of HIPAA laws, Confidentiality, state licensing regulations, CPR, AED and First Aide, other trainings as required.
Education, Experience:
Current LPN license in state of practice. Knowledge of continuous improvement techniques. Awareness and understanding of regulating bodies (CARF, Office of Mental Health, DDAP). Valid driver’s license.
