JOB SUMMARY:
Provides direct and indirect care services that meet the psychosocial, physical and general aspects of care; meets the communication needs of patient and family; provides care that reflects initiative and responsibility indicative of professional expectations, under the supervision of a registered nurse. Maintains regulatory agency requirements, nursing and hospital policies, procedures and standards. Communicates with physicians and team members about changes in patient’s clinical condition, including results of diagnostic studies and symptomatology. Is able to respond quickly and accurately to changes in condition or response to treatment.

DUTIES AND RESPONSIBILITIES:
Licensed Practical Nurse
Performs patient care responsibilities considering needs specific to the standard of care for patient’s age. Treats patients and their families with respect and dignity. Identifies and addresses psychosocial, cultural, ethnic and religious/spiritual needs of patientsand their families. Interacts professionally with patient/family and involves patient/family in formation of the plan of care. Performs all aspects of patient care in an environment that optimizes patient safety and reduces the likelihood of medical/health care errors. Adequately assesses and reassesses pain. Utilizes appropriate pain management techniques. Educates the patient and family regarding pain management. Knowledge of medications and correct administration based on age of the patient and patient’s clinical condition, to include IV medications as specified by facility. Follows the five medication rights and reduces the potential for medication errors. Correctly obtains laboratory tests; such as urine pregnancy tests, blood glucose tests, PT/PTT, and others as indicated by the physician. Able to interpret results of laboratory tests accurately, taking appropriate action and notifying RN and physician of test(s) results. Establishes that the patient meets discharge criteria. Provides discharge instructions. Carries out medical and surgical asepsis during treatments and special procedures. Meets current documentation standards and policies. Responds appropriately to patient in an emergent situation. Manages and operates equipment safely and correctly. Coordinates and supervises patient care under the direct supervision of an RN. Consults other departments, as appropriate, to provide for an interdisciplinary approach to the patient’s needs. Communicates appropriately and clearly to charge nurse, co-workers and physicians. Maintains a good working relationship both within the department and with other departments. Performs other duties as assigned within the LPN scope of practice.
