The Social Worker - LICSW is a fully integrated Mental Health Professional and core member of the primary care team that works under the direction of the mental health faculty provider and primary care team. The role of the BHC is to support the primary care team in the early identification and treatment of mental health conditions within the defined clinic population.
Social Worker - LICSW OverviewKey responsibilities include but are not limited to:
Acting as a fully integrated Mental Health Professional and core member of the primary care team that works under the direction of the primary care team, sees patients in assigned clinic(s).Mental Health Home services, providing team support / consultation, and population health.See patients being seen by medical providers within clinic to provide brief interventions for mental health problems, assistance with engaging in healthy behaviors to address chronic illnesses, crisis assessment and intervention, patient intervention per clinic protocols, other activities as they come up in clinic.Functions as an embedded member of an integrated and consultative interdisciplinary teamResponds to the needs of the team. Provides expertise, direction, consultation, and leadership to the CHW and SW CC.Engages in Population Health strategies and conducts panel management. Fulfill expectations for how to use and maintain the patient registry. Remain up to date on requirements of Mental Health Home certification and practice standards.Enter, update and review the patient registry to conduct panel management.Responsibility for developing and nurturing relationships with other community and social support providers to aid in effective referrals and timely access to services.Demonstrates ability to provide applicable care/service adjusting approaches to reflect developmental level of population served.Minimum Qualifications:Licensed Social Worker (LICSW), or licensed to practice independentlyMN state licenses, or eligible to be licensed in the State of Minnesota and eligibility to become a credentialed provider with insurance entities.2 years’ experience post licensure.Experience in providing diagnostic assessments and direct evidence-based clinical services to children and adults.Diagnostic evaluations, Psychotherapy, Family Therapy, Health and behavioral coaching, Individual and group coaching, Good interpersonal and organizational skills.Location: 580 Rice St, St Paul, MN 55103 & 2020 E 28th St, Minneapolis, MN 55407
