Position: Licensed Vocational Nurse (LVN) – Home HealthLocation: Sunnyvale, CA
We are seeking a compassionate and skilled LVN to provide top-quality care to our patients in their homes. As a Home Health LVN, you will:
- Deliver personalized nursing care to a diverse patient population in the comfort of their homes.- Collaborate with an interdisciplinary team to develop, implement, and evaluate individualized care plans.- Monitor and document patient conditions, including vital signs, medication administration, and wound care.- Educate patients and their families on managing illnesses and maintaining health at home.- Communicate effectively with physicians, case managers, and other healthcare professionals to ensure the best possible outcomes for your patients.
Compensation: $45- $55/hour, depending on experience.
Requirements:
