Role Overview:
The Life Safety Code Surveyor is responsible for conducting on-site surveys in skilled nursing facilities, nursing homes, ambulatory surgery centers, Intermediate Care Facilities for the Individuals with Intellectual Disabilities, and ESRD (Dialysis Centers) to determine compliance with federal certification, state licensure, and National Fire Protection Association (NFPA) requirements where applicable. This position reviews and evaluates fire protection requirements and Emergency Preparedness plans. The Life Safety Code surveyor must have at least a bachelor's degree and/or be qualified in one of the following professions: Hospital administrator; Industrial hygienist; Laboratory or medical technologist, bacteriologist, microbiologist, or chemist; Medical record librarian; Nursing home administrator; Nutritionist; Pharmacist; Physical Therapist; Physician; Qualified Intellectual Disabilities Professional; Sanitarian; Social worker; building/facility maintenance; or fire safety and/or fire codes. This position requires in-state travel including some occasional overnight stays in lodging near survey assignment sites. Travel expenses are reimbursed. This is a field-based position. Applicants living in Lake, Porter, LaPorte, St. Joseph, Newton, Starke, or Marshall county will be considered.
Salary Statement:
The salary for this position traditionally starts at $47,320.00 but may be commensurate with education or work experience.
A Day in the Life:
The essential functions of this role are as follows:
+ Conduct periodic surveys and complaint investigations as required by State and Federal policy and defined protocols.
+ Serve as team leaders for surveys, assigning survey tasks, conducting entrance and exit conferences, posting notifications and submitting survey paperwork timely.
+ Identify deficient practices and determine appropriate scope and severity in the provision of healthcare and related services.
+ Conduct on-site investigation of complaints to identify deficient practices and determine appropriate scope and severity in the provision of healthcare and related services.
+ Document findings on the survey report in accordance with Principles of Documentation and submit reports within the required timeframes.
+ Complete administrative duties including, but not limited to travel vouchers, PeopleSoft Time and Labor, schedules, checking and responding to emails, and computer updates.
+ Complete a quality review of all assigned survey reports and make recommendations to ensure compliance with Principles of Documentation and ISDH Policies and Procedures.
+ Organize and prioritize workload to ensure review and submission of survey reports in prescribed time frames.

You must meet the following requirements to be considered for employment:

+ Specialized knowledge of operation and administration of applicable healthcare entities including applicable rules and regulations.
+ Working knowledge/understanding of survey and investigative procedures; the ability to conduct surveys, inspections, interviews, and investigations; and to prepare reports documenting findings.
+ Practical knowledge of appropriate state and federal laws, state rules and federal regulations for licensing of type of entity being surveyed.
+ Practical knowledge of and the ability to review and analyze facility records and documents for compliance with requirements.
+ Ability to observe and recognize potential adverse health and safety conditions, a violation of a law, rule, or regulation.
+ Ability to communicate verbally and in writing with individuals and in group situations.
+ Perform quality review of survey reports and various program requirements with specialized training.
+ Understanding of and ability to work as a member of a team.
