POSITION SUMMARY The link+care coordinator will provide intensive medical case management to HIV + clients who are either newly diagnosed or returning to care after a year. Coordinator will work directly with clients to overcome barriers to care. Coordinator will assist in community outreach, education and testing. Coordinator will assist in HIV advocacy projects.  POSITION QUALIFICATIONS Minimum Education Bachelor’s degree in a health related field
Minimum Experience Two years’ experience working in health related or social service field
