JOB PURPOSE:
Responsible for assisting and guiding patients considered as a potential kidney living donor and their caregivers through the continuum of care and supporting their decision-making processes. Gathering required information from patients or external sources to enable a thorough clinical evaluation. Assembling appropriate clinical information for prospective review by the multidisciplinary team or treating physician. Coordinating additional testing procedures as directed by the team or treating physician. Educating patients about the donor evaluation process prescribed treatment plan through donation and follow up. Escorting patients to support services, imaging and assisting as needed. Providing community education and outreach services that promote this patient centered care model. Coordination of patient care between potential living donors, hospital entities and the transplant team.

Qualifications:

MINIMUM EDUCATION REQUIRED:
High School


MINIMUM EXPERIENCE REQUIRED:
1 year of customer care experience.


MINIMUM LICENSURE/CERTIFICATION REQUIRED BY LAW:
High School diploma


ADDITIONAL QUALIFICATIONS:
Knowledge and experience in using the EMR system.
Previous living donor preferred


KEY RESPONSIBILITIES:
1. Facilitates clinical workup by determining the most appropriate resource/service for each patient;
2. Collects, organizes, and analyzes pertinent patient information to assist in treatment planning decisions;
3. Presents appropriately organized information to multidisciplinary team (or treating physician) for evaluation plan determination;
4. Communicates treatment plans to patients and assists in scheduling procedures as needed;
5. Provides appropriate patient/family education throughout the care process;
6. Links patients and families with services across care continuum;
7. Communicates appropriately with physicians and clinical staff;
8. Monitors program performance and general concerns within the Mason Transplant Clinic
9. Facilitates process mapping for the living donor patient's day
10. Manages referral questionnaire and contact, as needed, including calling, scheduling, etc.;
11. Facilitates donor evaluation day, post operative day and post follow up days
12. Supports marketing and public relations initiatives in relation to patient experience
13. Maintains contact with community resources and supports Piedmonts priorities for community outreach.
14. Compiles testing as needed.
15. Assists with admits/Discharges.

KNOWLEDGE, SKILLS, ABILITIES
Leadership skills including time management, verbal and written communication, active listening, decision making, priority setting and organization.
Must be self-directed, use independent judgment and demonstrate effective team skills.
Current working knowledge of healthcare systems, Radiology services and the managed care environment.
Working knowledge of the Piedmont Hospital system.
Knowledge and experience in using the EMR system.
