Job Description:
  The Long Term Care Liaison LVN positions works closely with the Clinical team, patients, families and nursing facilities.  
Qualifications: Graduate of an accredited college or university or two years of experience working in hospice and/or home health patient care Must be a Licensed Vocational Nurse (LVN) in the State of Texas Experience with long term care - required Must have good oral communication and writing skills Must be able to speak well in public Must be able to practice the principles of good public relations Must have a current state driver's license Must have reliable transportation Must have a current auto insurance policy Clean background check and drug screen
