Long Term Care Navigator
Overview

Facilitates the identification of VNSNY Home Care patients who may be in need of long term care (LTC) services as part of their discharge plan. Communicates with clinical team to initiate the long term care planning process and helps navigate and track these patients through all stages of the process, including identification, outreach, referral and enrollment. Acts as resource for and primary point of contact between CHHA clinical team, Revenue Cycle and LTC program. Works under moderate supervision.


Responsibilities
Manages list of patients who are potentially in need of long term care services based on clinical indicators.Communicates directly with CHHA clinical staff and management to notify them of patients who meet clinical indicators for long term care and trigger discussion regarding long term care eligibility and options.Assists in identifying appropriate LTC planning options based on patient’s insurance coverage.Communicates directly with patients and caregivers, as needed, to provide education and assistance to facilitate the transition process to long term care. Serves as resource and point of communication between CHHA clinical and management staff, Revenue Cycle, and MLTC to track and facilitate the movement of patients through the CHHA to LTC enrollment process.Assists with outcomes reporting and other analysis of data related to the transition of patients from CHHA to LTC.Participates in special projects and performs other related duties, as required.
Qualifications

Education:  Associates Degree in related field OR equivalent work experience required. 

Experience:  Minimum of two years in healthcare or human services, preferably in a role that included communication with clinicians, other providers and care recipients/caregivers. Working knowledge of healthcare insurance and long term care eligibility and planning options required. Excellent communication, coordination and customer service skills required. Proficient personal computer skills, including MS Word, Outlook, and Excel required. Bilingual English/Spanish a plus.
