Community Healthlink (CHL) is looking for a part-time (24 hrs/wk) Acute Care LPN's in Northern Worcester County.
This position offers an opportunity to join a dynamic, engaged, and caring team that is focused on supporting clients and staff in developing their potential.
Assess the physical and psychosocial status of patients in all settings by obtaining appropriate patient histories, administering physician/primary care provider orders, nursing assessment, medication administration reconciliation, take and monitor vitals of clients on the unit, medication administration and documentation.
Renders and supervises care of clients on the unit.
Monitors medication and consultation with prescribers.
Provide appropriate health care and coordinates referrals as prescribed by providers.
Participates in the evaluation of client's condition and progress with team members.
Promote education and adherence to prescribed treatment protocols.
Facilitates wellness and medically related psychoeducational groups to individuals.
Records all client information within electronic medical record system daily.
Assists RN on daily basis.
Offer nursing consult on emergency service evaluations.
Work with clients to coordinate care with their PCP, psychiatrist, and other professionals including but not limited to medication refills and f/u medical appointments.
Demonstrates ability to communicate effectively and professionally with clients, staff, and providers.
Demonstrates proficiency in completing electronic documentation in compliance with grant requirements, agency, state, and federal guidelines and policies.
Knowledge of medical, substance abuse and mental health nursing
Respects diverse views and approaches, demonstrates Standards of Respect, and contributes to creating and maintaining an environment of professionalism, tolerance, civility and acceptance toward all employees, patients and visitors.
Qualifications:
Grade N04: Acute Care LPN: Graduate of an accredited school of Nursing with current license as a Licensed Practical Nurse in the state of Massachusetts.
Previous clinical experience working with substance abuse and mental health.
Demonstrated strong work ethic, ability to work independently, and ability to contribute to team objectives required.

Current valid US-issued driver's license and ability to provide registered, inspected and insured automobile for work related purposes including transporting clients in your own vehicle
