+ JOB DESCRIPTION DETAILS + Job Summary : + Licensed Practical Nurse at CalvertHealth provides nursing care, under the direct supervision of a registered nurse pursuant to the Maryland Nurse Practice Act and the policies of our hospital. This specific position will be to work within the value based care staff and clinical staff to identify, schedule, coordinate and perform Annual Wellness Visits for CalvertHealth’s CMS population. + Education : + Completion of an accredited LPN Program and LPN - State Licensure + Registration/Certification/Licensure : + + Current Maryland State LPN License in good standing + CPR, BLS Certification + NRP Certification within 6 months + Experience : + + 1 - 2 years of experience as an LPN preferred + Experience in primary care, geriatrics, or chronic disease management preferred + Familiarity with Medicare wellness visit guidelines and documentation requirements + Other Requirements : + Maintains unit-specific and hospital competencies, mandatory learning, and any clinical certifications required in accordance with the Staff Education and Training policy GA-057 and/or any other department requirements. + Proficiency with HER systems and clinical documentation + Strong communication and patient engagement skills + Attention to detail and ability to follow clinical protocols + The Licensed Practical Nurse (LPN) contributes to the provision of high-quality, cost-effective, healthcare as a provider of direct and indirect patient care. + The LPN contributes to the nursing process in a team relationship with the Registered Nurse. Under the supervision/or with assistance from an RN the LPN will, assess, plan, implement, evaluate, and document the daily care of assigned patients.
