Key Responsibilities:

Administer medications via oral, injectable, and self-administered routes according to service line standards

Conduct physical and mental health nursing assessments, including psychiatric screeners

Complete documentation accurately and timely, adhering to clinical and compliance standards

Perform MAR reviews and monitor housing medication stocks; prepare pass medications as needed

Address client questions regarding medication and coordinate refill requests with providers

Room clients and gather medical data including vitals, urine samples, and bloodwork

Support documentation of nursing-led interventions, educational groups, and safety plans

Assist with lab and phlebotomy tasks as needed

Act as a resource on infection control, hygiene, and health-related concerns within the facility

Participate in scheduled team meetings and collaborate with nursing leadership

Perform other related duties as assigned
 

Education and/or Experience Qualifications:

Graduation from an accredited practical nursing program

Current licensure as a Licensed Practical Nurse (LPN) in the state of practice
 

Required License/Certification (one of the following):

Valid LPN license in the practicing state
 

Additional Qualifications:

Strong clinical documentation and communication skills

Comfortable using electronic health records (EHR)

Able to manage multiple tasks in a fast-paced care environment

Committed to patient confidentiality and HIPAA compliance

Demonstrates sound clinical judgment and adaptability
