Summary:

Provides prescribed medical treatment and personal care services to ill, injured, convalescent, and disabled persons in hospital, clinic, assisted living and nursing facilities by performing the following duties personally.  Responsible for ensuring that all critical tasks are fulfilled on a timely basis.


Critical Tasks:

Ensures that patients and residents are comfortable and well cared for according to state and federal regulations as well as contemporary nursing practices. Interviews patients to determine medical problems/conditions and documents this information in patient charts on a highly accurate basis.Obtains and records patient's vital signs and weight.Assigns direct care personnel daily assignments while ensuring that staff capabilities meet or exceed all patient and resident needs.Ensures proper patient care by all CNA’s during assigned shifts.Provides medications to patients only as specifically ordered and documented with 100% accuracy in terms of medicine, dose and timing.Reviews physician's orders, lab requests, or follow-up needs with patient.Maintains medical records that are complete and accurate.Dresses wounds and gives enemas, douches, alcohol rubs, and massages. Provide comprehensive wound care and treatment to prevent skin breakdown. Applies compresses, ice bags, and hot water bottles.Charts patient and resident data in compliance with Medicare requirements while accurately noting abnormal findings and changes in patient and resident condition.Observes patients and reports change in condition to nurse in charge.  Administers specified medications, orally or by subcutaneous or intermuscular injection, and notes time and amount on patients' charts as directed by medical staff.Collects samples such as urine, blood, and sputum, from patients for testing. Performs all duties only within competency, certification and licensure limitations.May perform functions of IV administration and wound vacuum assisted closure upon valid medical orders if professionally trained and certified.Performs all duties with the highest level of cleanliness and when appropriate, sterility.Sterilizes equipment and supplies.Transcribes chart orders and ensures that all ordered medications are available as prescribed.Records food and fluid intake and output.Bathes, dresses, and assists patients in walking and turning.Maintains supplies, including stocking and ordering new supplies and disposing of expired or damaged supplies.Responds to patient inquiries by evaluating and responding to the patient's problems and questions.Follows up with patients on lab results, including the scheduling of any additional tests.Coordinates referrals for patients through insurance and other physician offices.Demonstrates ability to regularly change priorities to accomplish all tasks despite frequent interruptions.Demonstrates ability to clearly communicate, both orally and in writing, while performing all essential functions.Performs other special projects and duties as assigned.

Required Qualifications:

Graduate of an accredited Licensed Practical Nurse program from an accredited university or college.AZ Licensed Practical Nurse state licensure in good standing.Current and continuing CPR and AED training and certification.Demonstrated ability to relate to diverse cultures and specifically the Gila River Community and/or other Native American cultures.Employment at the Caring House requires: AZ Department of Public Safety Fingerprint Clearance Card.
