Position: Licensed Practical Nurse (LPN)The Grand Rehabilitation and Nursing Center at Pawling, located in Pawling, NY, is actively seeking LPNs for all shifts. We offer full-time, part-time, and per diem positions to suit your schedule.
LPN Responsibilities: Implement and coordinate care delivery in collaboration with physicians and care associates. Conduct regular health and wellness checks for residents. Assist in developing and updating individualized service plans for residents. Maintain open communication with residents, families, and physicians regarding their care needs. Utilize the nursing process (assessment, planning, implementation, evaluation) to ensure optimal resident care. LPN Requirements: Current and valid NYS LPN License in good standing. Ability to work effectively in a team-oriented environment. Prior experience in a nursing home or hospital is preferred but not required. What We Offer: Immediate hiring opportunities.
