Hourly pay range: $23 - $26

As directed by the RN, assists with the monitoring and coordination of the delivery of health care services for individuals with developmental disabilities who are in a structured day program. 

ESSENTIAL FUNCTIONS/RESPONSIBILITIES:

Assists with monitoring and coordinating the delivery of preventive routine health care, therapies and programs for individuals with developmental disabilities Adheres to and assists with ensuring facility compliance with all Physician, NP, PA, etc. orders. Assists with coordinating medical program needs with community services, medical providers and other related agencies. Assists with ensuring adequate completion of health related exams, appointments, laboratory/diagnostic testing as ordered or required. Safely transports individuals to and from appointments and community outings if necessary. Assists with the monitoring of medication administration in the facility. Administers medications, treatments and supplements as assigned. Assists with providing health services training for staff. Participates as an active member of the Interdisciplinary Team. Maintains compliance with and assists with ensuring facility compliance with all applicable State/Federal regulations and agency policy and procedure. Maintains communication with Medical Liaison, Program Manager, Program Administrator, Service Coordinator and other staff as necessary. Monitors that transportation and all medical specific procedures are being performed adequately by direct care staff Performs CPR and Strategies for Crisis Intervention and Prevention (SCIP-R) Complies with all agency policies and procedures Other duties as required.

 

MINIMUM QUALIFICATIONS:

Current NYS licensure to practice as a Licensed Practical Nurse NYS Drivers License that meets agency policy Ability to meet physical requirements of the position when performing the following: Standing, walking, sitting, bending, stooping, squatting, kneeling and climbing Lifting requirements of 50lbs Ability to lift and transfer individuals manually and mechanically as required by prescribed program/plan Ability to move, reposition, and place individuals in pieces of equipment Physical agility and ability to react to emergency situations, including maintaining Strategies for Crisis Intervention and Prevention (SCIP-R) certification as per regulatory guidelines CPR certification required
