Job Description​

We are looking for a highly qualified and career-minded Licensed Practical Nurse (LPN) to work as a LPN Charge Nurse in our 57 bed skilled nursing center.  Must also be willing to float to our Assisted Living unit.

This is a full time day shift position. Every other weekend is required.

Our LPN's provide direct nursing care to residents and supervise the day-to-day nursing activities performed by CNA's. Specific job responsibilities include, but are not limited to:

Meeting with assigned nursing personnel to plan the shifts' services, programs, and activities. Overseeing the activity of assigned nursing assistantsAdministration of professional nursing services and medication to residents. Ensuring residents receive services in accordance with their care planAssisting with new admissions and dischargesConsulting with residents, family members, and physicians in planning the resident’s care, treatment, and rehabilitationMedication administrationVisiting residents to observe and evaluate their physical and emotional status.Notifying the resident’s attending physician and next-of-kin when there is a change in the resident’s conditionReporting problem areas to the RN Supervisor or Clinical DirectorDemonstrating excellent customer service skills and professional demeanor at all times
