Schedule: Monday–Friday, 8 AM–5 PM (no weekend shifts)

Compensation & Benefits Salary: $56,000–$64,000 annually Health Benefits: Multiple major medical plan options, dental, vision, spousal coverage Retirement: 401(k) Insurance: Basic, voluntary life insurance & AD&D Leave: Paid Time Off (vacation, sick, and public holidays) Disability Coverage: Short-term & long-term Professional Growth: Ongoing training and development Culture: Inclusive, supportive atmosphere with employee recognition programs Key Responsibilities Develop, review, and revise care plans Deliver nursing care per physician orders and payer guidelines Monitor and document patient responses and condition changes Provide education and oversight for clients, families, and staff Participate in education programs, clinical meetings, and quality reviews Oversee patient and staff assignments and manage referrals Initiate discharge planning and complete timely discharge summaries Conduct supervisory Home Health Aide visits as required Provide PRN coverage, rotate weekend support, and be on call Engage in quality improvement initiatives and chart audits Collaborate daily with the Director of Nursing for patient status updates Travel to patient homes and outreach sites as needed Ensure compliance with agency policies, procedures, and regulations Perform additional duties as assigned by supervisor Required Qualifications Active Missouri or compact LPN license Minimum one year of home health experience and at least one year practicing as an LPN Valid driver’s license, reliable vehicle, and current auto insurance Current CPR certification Ability to lift up to 50 pounds Effective verbal and phone communication skills Strong organizational skills and ability to manage tasks independently Why Join Phoenix in SpringfieldAt Phoenix, you'll lead a dedicated clinical team in a role that blends leadership with hands-on care.
