Lutheran Hospital's 5D Unit cares for a variety of post-operative patients in the areas of orthopaedics, spine, neurology and bariatrics. Some procedures patients have undergone include total joint replacements, lumbar and/or cervical decompressions, fusions, and laminectomies. This unit also treats general med/surg patients and can telemetry and ongoing cardiac monitoring. This is an excellent opportunity for an RN to join a team that is active in shared governance, is involved in community outreach, and has created a highly engaged work environment. **A caregiver in this position works Nights, from 7:00pm-7:30am or 11:00pm-7:30am.** A caregiver who excels in this role will: + Provide direct nursing care in accordance with established healthcare organization policies, procedures, and protocols. Implements and monitors patient care plans. + Monitor, record, and communicate patient condition as appropriate. + Monitor and document patient's condition and effectiveness and timeliness of care and/or interventions. + Evaluate, update, and revise plan of care to facilitate achievement of planned and expected outcomes. + Serve as a primary coordinator of all disciplines for well-coordinated patient care. + Administer prescribed medications and treatments in accordance with approved nursing protocol. + Note and carry out physician and nursing orders. + Assess and coordinate patient's discharge planning needs with members of the healthcare team. + Assess the educational needs of the patient and significant others considering cultural issues and other special concerns. + Provide teaching and educational material as appropriate. + Validate and document patient and family understanding of teaching. + Maintain professional growth and development through ongoing formal and informal education and clinical nursing study. Minimum qualifications for the ideal future caregiver include: + Graduate from an accredited school of professional nursing. + Licensed Practical Nurse (LPN) licensure and current registration in the State of Ohio. + For specific areas, must be certified in medication administration and IV therapy as approved by the Ohio Board of Nursing. + Basic Life Support (BLS) certification required within one year of service. + The Licensed Practical Nurse must have a minimum of one year clinical experience within the previous three years + OR must have graduated within the previous six months of hire.
