This position supports the clinical Lead and staff RNs in the care of patients at the Hildebrandt. The LPN utilizes their complete scope of practice and is an integral member of the Interdisciplinary Care team. Under the supervision of an RN, the LPN will at times carry their own patient assignment, and at other times assist the RNs with theirs.

STATUS: Full time - 40 hrs/week
LOCATION: Hildebrandt Hospice Care Center
SCHEDULE: Mon - Friday 9 am-5:00pm.

RESPONSIBILITIES
• Provide all aspects of patient care within scope of practice
• Take and give change of shift report- Provide education and support to patients and families- Complete all required Healthstream courses and in-services- Document GIP notes in Netsmart, medications administered in MARs, complete weekly IDGs as requested- Other tasks as needed e.g. restocking nurse supply closets, put supplies away

EDUCATION:

LICENSES / CERTIFICATIONS: 

BLS - Basic Life Support - American Heart Association (AHA), LPN - Licensed Practical Nurse - New York State Education Department (NYSED)

PHYSICAL REQUIREMENTS:

M - Medium Work - Exerting 20 to 50 pounds of force occasionally, and/or 10 to 25 pounds of force frequently, and/or greater than negligible up to 10 pounds of force constantly to move objects; Requires frequent walking, standing or squatting.

The listed base pay range is a good faith representation of current potential base pay for a successful full time applicant.
