Role and Responsibilities Administer practical nursing care to patients in accordance with physicians orders under the direction and supervision of a Registered Nurse. Essential Functions Under the direct supervision of an RN Case Manager, and within the scope of practice for a Licensed Practical Nurse: + Provide health education according to the teaching plan established by the RN. + Provide routine LPN nursing care as outlined in the nursing care plan and within the scope of practice for an LPN. + Observe signs and symptoms, and report patients reactions to treatments including drugs and changes in the patients physical or emotional condition, to the physician and RN Case Manager. + Document any services rendered to the patient, changes noted in the patients condition, and/or family and home situations. + Document communication with RN Case Manager and physician. + Other duties as assigned.
