Schedule: Every Saturday and Sunday 8a-8p
The Community Hospice has a wonderful opportunity for a Weekend Track LPN to work every Saturday and Sunday as a member of our Hospice Team caring caring for patients and families in the homecare and nursing home settings in the Schenectady/Albany/and Rensselaer areas.

Coordinate closely with our RN Supervisor and RNs to provide nursing and personal care services as a part of a comprehensive plan of care to provide added support to patients and familiesThis is an ideal opportunity for someone who enjoys working closely with a team (our teams include RNs, Aides, medical directors with specialized expertise in end of life care, social workers, chaplains, complementary therapists and volunteers). Training provided in end of life care.What you will need:
Valid NYS LPN LicenseMinimum 1 year LPN experience requiredPrior hospice/ home care experience preferred, but not requiredStrong computer skillsOrganization and communication skills and ability to work as part of a teamCommunity Hospice provides a thorough orientation, preceptors and a supportive work environment. Pay Range for full-time or part-time (Non-Weekend Track): $24.00-$31.10 Pay is based on experience, skills, and education. Exempt positions under the Fair Labor Standards Act (FLSA) will be paid within the base salary equivalent of the stated hourly rates. The pay range may also vary within the stated range based on location.”
